As in all fields in medicine, specialty areas have developed in ob/gyn. A resident may spend time with a faculty member in her program and decide that she especially likes one aspect of the field. After completing the preliminary four years of general ob/gyn training, she can enter a fellowship. Fellowship areas in obstetrics include maternal/fetal medicine and high-risk obstetrics. Areas of concentration in gynecology include reproductive medicine, advanced laparoscopy, oncology (cancer), and urogynecology.

Often, the examining physician cannot tell what organ lies behind the protruding vaginal skin. It is possible to have a good idea based on the location and shape of the defect. Some physicians will order tests, such as CT scans or MRIs to get a better sense of the structures that lie behind the prolapsed tissue. Other physicians operate without knowing for sure what is there. They will correct whatever defects they can find. Many women with enteroceles notice that their vaginas are shorter than they used to be.

Not every physician elects to do a fellowship. Those who do, spend an extra year or two focusing on a particular aspect of their field, allowing them to become an expert. When new techniques are being developed or research is being carried out, the fellows are usually involved with a senior physician. This specialty training can be either accredited or not. If the specialty field is well established, accredited fellowships are usually available. Sometimes, a specialty field is very new, so no accreditation efforts have been instituted.
